Our team collaborates with our clients to determine the best plan possible for all of their healthcare needs.

  Our Mission

Our mission is to create a collaboration of trust while elevating the health and well-being of our patients along with the community partners we serve.

  Our Vision

Our vision is to be creators of change by partnering with our community, clients and care team members. We see this partnership as the key to optimal health and wellness in our community. Furthermore, where there is illness and poor health, we will work in tandem with the client to achieve their full potential and improve their well-being.

Welcome to Partners Healthcare

At Partners Healthcare, our primary aim is to partner with you and other medical providers, as needed, to enhance and ensure your health and physical well-being. This means we take a proactive, collaborative and holistic approach to everything we do, from physical exams to the administration of medications to mental health and other evaluations.

Our “partners” include other physicians and specialists as well as our patients themselves, with whom we work to identify healthcare issues and needs and then to develop a holistic plan to address specfic issues and medical needs. Working together, we’ll evaluate your situation then work with you and, if needed, our other partners to get you the best care available.

Meet Our Team

Scott Seitz

Scott Seitz

MSN, APRN-BC

I have been in the medical field for over 40 years. I have a vast array of experiences, including emergency medicine, hospital medicine, primary care, occupational medicine and addiction medicine. I felt called to make a difference in the healthcare delivery to our community and opened Partners’ Healthcare to make that happen. I have a strong sense of ethics and am driven by my faith.

I feel I do things differently from other medical professionals and my goal is to work myself out of a job. If I have cared for my clients in such a way that they are completely healthy and do not need me, then I have done my job. I believe there is more than one way to health, and I enjoy exploring those avenues with my clients. I believe in holistic care and have found that connection is key to living a healthy, balanced life.

I have an amazing wife, Kelly, of over 30 years. I have three beautiful children, Ashleigh, Rebecca and Erik. I also have three fantastic in-law children and two adorable grandchildren. Family time is my favorite pastime. Outside of that, I love to be outdoors, and fishing makes me very happy. I also enjoy camping and taking photographs.

I have witnessed substance use disorder wreak havoc on our community and I desire to be part of the solution to improve the lives of anyone who is still experiencing active addiction.
